What are the 1099 form layouts?

Each sheet contains information for two recipients. Forms are sold in packages of 50 (8-1/2″ x 11″) sheets yielding 100 individual 1099 forms. 2-Up forms are collated for envelope insertion.

Each sheet contains 3 forms per sheet. May contain the three of the same 1099 form part or a mix. Available as a pre-printed form or as a blank form with perforations and backer copy.

4-Up sheets contains four boxes or rows of 1099 forms with recipient and payer copies. Available as a pre-printed form or as a blank form with perforations and backer copy.

Blank forms are available for use with your software and contain pre-printed backers. Available for various 1099 forms (i.e. 1099-DIV, 1099-INT, 1099-R).
